For Our New Pregnant Clients
If you’re coming for your first visit and are pregnant, please fill out the New Client forms above in addition to the Client Pregnancy Intake form and bring them in with you.
Insurance Billing
If we are billing insurance for you, please complete our Insurance Pre-approval form. For massage after a motor vehicle accident, please also fill out out our Client Health Info Form and Client Status Report.
